Athletic Packet

All athletes must complete an Athletic Packet to participate.

Parents must complete the packet and hand it in at their school site. No athletes may be allowed to practice or compete without a packet on file in the office. One packet will be utilized for all sports. Filling out a packet for each sport is unnecessary unless information needs to be updated. 

Please download the athletic packet, print, sign the appropriate pages, and submit it to your school site office.
