Chronic Health Conditions
Health Care Plans:
Students with health conditions will need updated health care plans before school starts each August. These plans are conveniently located below. All forms are in a PDF format that can be completed online and emailed directly to the District Nurse at [email protected].
“Medication Administration Request” forms are also online and must be updated annually. You will need to complete this form and have your child’s physician authorize the medication. Remember all medications (even those over-the-counter) require a Medication Administration Request. If you child has a special diet, please have your physician complete the Meal Accommodation Form.

Asthma:
A condition in which a person's airways become inflamed, narrow and swell, and produce extra mucus, which makes it difficult to breathe.
If your child has asthma and needs an inhaler at school please complete the Asthma Action Plan and Medication Administration Request.
Epilepsy/Seizures:
Based on the type of behavior and brain activity, seizures are divided into two broad categories: generalized and partial (also called local or focal). If you child has a history of seizure activity please complete the Seizure Action Plan.

Allergies:
An allergy is a heightened sensitivity to a foreign substance that causes the body's defense system to overreact when defending itself. The severity of an allergic reaction can vary from mild discomfort to life-threatening situations.
If you child has allergies please complete the Allergy Action Plan. If your child is allergic to food please complete the Meal Accommodation Form. In addition, if specific medications are necessary, such as antihistamines or epinephrine please have your physician complete the Medication Administration Request.
Diabetes:
Students with either insulin-dependent Diabetes, or Diabetes that is controlled with oral medication will meet with the school nurse and school site team annually to review and update healthcare plans. Each school year, we require that your physician provide the school district with updated medical orders. Below is a form Sample Diabetic School Orders, however, most Physician's prefer to use their own approved document.
