    The Paso Robles Joint Unified School District Home Hospital Program is designed to provide instructional continuity to students who are temporarily unable to attend their regular school of enrollment due to: physical illness/injury, mental illness, or chronic health impairment.  Home/Hospital is a short-term instructional service mandated by state law with specific guidelines for program implementation and delivery. These educational services enable students to continue their academic work and to remain current with their peers as they prepare for their return to school.

    In order to qualify for Home/Hospital services, the student must be expected to be absent from school for a projected period of 10 or more consecutive school days.  To apply for Home/Hospital Services obtain a Home Hospital Request Form (from the school or District website, both English and Spanish available below).  Complete the parent/legal guardian section and forward the form to the appropriate medical professional. The completion of this form authorizes Paso Robles Joint Unified staff to communicate with your medical professional.   Please note that failure to sign this release of information may result in denial of Home/Hospital Services.

    The medical professional and or parent/guardian should return the completed form to the student’s school site for review. Upon receipt of the form, the school site administrator/counselor will schedule a meeting with the parent/guardian to review the Home and Hospital Instruction Program manual. The referral form once signed off by the student’s school site, is sent to PRJUSD Student Services Department for review and approval.  Please note, submitting the referral form does not guarantee Home and Hospital Instruction Program services. The student’s case will be reviewed and if the service is determined to be appropriate, the request will be approved. In some instances, modifications to the school day will be recommended prior to assigning Home and Hospital Instruction Program services, as it is our goal to keep students in attendance whenever possible so they may achieve student success.

    Once Home and Hospital Instruction Program services have approved a teacher will be assigned to provide services to the student. Paso Robles Joint Unified School District requires that a responsible adult (18 years of age or older) be present throughout the duration of the time the teacher is with the student.   Students will be required to work on assignments independently as well; please assist your student in completing assignments, as failure to work independently will result in students falling behind in their course work.

    Students with a disability who may receive instruction at home and who are receiving services through either an individualized education program (IEP) pursuant to the Individuals with Disabilities Education Act (IDEA) or a Section 504 plan pursuant to Section 504 of the Rehabilitation Act undergo a significant change in placement when their receipt of education is changed from school to home. Thus, the IEP team or Section 504 team is required to meet to reevaluate the student, review the terms of the IEP or Section 504 plan and revise as warranted, and determine the terms of the home instruction on an individualized basis in compliance with the IDEA’s and Section 504’s procedures governing the receipt of a free appropriate public education (FAPE).

    Please be aware that the maximum amount of time that a student can be assigned to Home/Hospital is 45 consecutive/cumulative school days. At that time, a review meeting will be held to determine the best educational program and services for your student.
